Risk questionnaire

Form that the insurer submits to the policyholder before contracting. It defines the extent of the duty to declare.

In practice

What is not asked cannot be reproached later: if the insurer did not submit a questionnaire, or did so generically, it cannot claim reservation. Keeping it completed is as valuable as keeping the policy.
